Where a CPAP device is purchased from us and you have received full or partial reimbursement pursuant to a group benefit plan (the “Plan”) from an insurer for it, we will refund the cost incurred (and paid in full) by you for any future mask or related accessories purchase if the same insurer determines the cost was not eligible for any reimbursement under the terms of the Plan provided the following conditions are met:
(i) You return the product to us, even if it was opened and/or used;
(ii) you give us written confirmation, dated within sixty (60) days of purchase, of the rejected claim from your insurer;
(iii) the rejected claim is from the Plan for which the purchase of your CPAP was approved.
The Rest Easy Promise can only be redeemed once per CPAP device purchased.
